Replying to Sg.{Arthur F. Sidgreaves - MD}1/E.22.10.35, this failure was actually due to a locking pin failing. It is mainly attributable to the part not being fitted correctly but the pieces are open to criticism in that defective fitting can produce such disastrous results.

We are making the necessary modifications and are circulating a Depot Sheet for those bumper bars already in service. Actually enquiries indicate that the Works have taken particular care to avoid this mistake on bumper bars they have fitted.
