The failure of a 'P' type oil cooler, attributing the cause to incorrect fitting.

Accordingly, we are of the opinion that the incorrect fitting of this cooler has been the primary cause of failure and that this failure may be disregarded from the point of view of reliability of the 'P' type oil cooler.

We shall, of course, require to be informed immediately if any other 'P' type cooler should fail and in that case the cooler in question should be returned to Rm{William Robotham - Chief Engineer}/Std. for examination.

The information and recommendations in Rm{William Robotham - Chief Engineer}/Std.2/JH. 10.5.38 (which was written before this 'P' cooler failure occurred) therefore still hold good.
