Dear Sirs,

We have to inform you of the failure in service on a car of one of your fog-lamps.

The failure occurred in the anchorage of the spring that carries the contact, which presses on to the cap of the lamp bulb.

Apparently the rivetting, which clamps the terminal, and the spring, to their insulating base, had broken away. This allowed the spring to work loose, with the result that it switched round. This in turn released the bulb, which fell out into the back of the lamp, and was smashed.

We are returning the lamp to you under seperate cover, for your inspection.
