The failure of a Phantom III push button switch, recommending a modification to the pigtail connection.

Re failure of Phantom III Push Button Switch, due to broken pigtail connection on the Carbon Contact. We have now carried out a test on a Push Button Switch using a carbon with a longer pigtail, enabling same to be slightly coiled. This switch was dismantled after completing 15,000 operations and on examination the pigtail was found to be 100% with no signs of fraying. We recommend that this modification be brought on production, and a retrospective action taken with regard to the existing switches. We have now rebuilt the switch and will carry out a prolonged test in order to ascertain the life of this longer pigtail.
