Eric Platford - Chief Quality Engineer}3/H18.1.24. and we should, as soon as ever possible, be in a position to issue instructions whereby we feel that this trouble can be finally overcome. We are also experiencing the spring washer failure on cars on test, and even the latest type on which the left hand nut is fixed (D.51067) still give trouble on test with broken washers. We have not yet had any experience with the latest type scheme as issued in your instructions BY/RD3/ML3.1.24, i.e. Alteration No. 1455/D.{John DeLooze - Company Secretary} consisting of two thin dished washers and one plain washer, but if it is considered that this scheme is a cure all, we recommend that instructions should be given at once for retrospective action. Please let us hear from you what you propose to do about the matter, as we should immediately like to inform our various Depots, as we anticipate a rush of enquiries and charges against us for rectifying this fault.
