We hand you herewith a Ph.III fan flange and blades which has failed on rig test. The piece No. of the flange is E.89254 and that of the blade is E.89255 - prints of same are attached. Will you please let us have at the earliest possible moment a report on the nature and probable cause of the failure, together with an analysis on the material of the blades.
