12-EX - PHANTOM ENGINE COUPLING.
The failure of the locking device fitted to the nuts is the same design as previously complained of. We have since had a new scheme of locking which was fitted to the coupling on 12-EX and which was O.K. after the 10,000 miles (see report).
This device has been overlooked when getting out the std. sheets and should be incorporated in the present coupling.
We have pointed out that the slots in the nuts G.74916/7 do not appear to be deep enough to be entirely satisfactory and recommend that the dia. of the hexagon should be increased if possible so that deeper slots could be made to ensure a more definite lock.
