Starter motor failures and the fitting of a three-washer scheme.

Eric Platford - Chief Quality Engineer} 14/HES.6.24. and your BY4/H20.6.24, dealing specifically with Starter Motor No. 727, our records do not show that this motor was fittedup with the three washer scheme, and from the dates of the failure we are almost certain that the three washer scheme would not be fitted. We have not as yet any knowledge of any breakages on the three washer scheme. The starter motor No. 885 referred to in your previous report was fitted up with the three washer scheme but these had not broken in this instances; the failure was due to the right hand nut having screwed back and locked the whole job solid.
