NOTE.—If you apply in person (with the old licence) at the Local Taxation Office, you need not fill up this form. Postage must be pre-paid if you make this application through the post. I apply herewith for the issue free of charge of a licence in replacement of the licence, of which the particulars are given hereon, and of which I am the holder, on the ground that the written entries or date stamp impression have become illegible through exposure to sunlight. In the event of the Council subsequently deciding that the illegibility is due to other causes, I undertake to pay on demand the fee of Five Shillings prescribed in the Regulations as payable in respect of a duplicate licence issued in replacement of a licence accidentally defaced. Strike out the paragraph not applicable. { I forward herewith the old licence, and request that the new licence be posted to me at the address given below.
