PHANTOM 111 - RE: BROKEN SERVO LEVER.

Sg.{Arthur F. Sidgreaves - MD}4/E.9.10.36.

I personally examined the servo which was returned owing to having failed in service, and found that the cause of the trouble was a crack which was produced in the lever during the forging operations.

The lever is a very simple one, and one cannot see any reason why a crack of this kind should have been produced in so simple a forging, but from an examination of our stock which was instituted immediately, we found there were others in a similar condition. They appeared to be all associated with the second delivery of levers, but I do not consider we should rely on this, and have instructed that it will be necessary for us to arrange for all the levers on chassis delivered to be replaced. There is a simple excuse that we can make to the owner in regard to this replacement, viz. that we are not satisfied with the rivets for connecting the two levers together, and that we are replacing the rivets by nickel steel bolts, as there seems a tendency for the rivets not to completely fill the hole.
